Seretide combination inhaler (contains the steroid fluticasone and longacting beta agonist salmeterol) to be given through a spacer device. The darker the inhaler, the higher the dose. Left hand has 50 mcg, middle one 125 mcg and right hand 250 mcg of fluticasone per puff, whilst they all contain the same 25 mcg dose of salmeterol.

Seretide combination inhaler (contains the steroid fluticasone and long acting beta agonist salmeterol) can also be given as a dry powder in a device called an accuhaler. It comes with different fluticasone strengths - 100, 250 and 500 mcg per puff, always with salmeterol at the same dose of 50 mcg per puff, but I only use the lowest dose as a dry powder.

Symbicort combination inhaler (contains the steroid budesonide and long acting beta agonist formoterol) can only be given as a dry powder in a device called a turbohaler. It comes in different combinations – 100/6, 200/6, and 400/12; the budesonide dose is 100 or 200, the formoterol dose 6 or 12 per puff. I only use the lower dose as a dry powder.

Stopping corticosteroid therapy
In autoimmune disease, clear end-points should be set before starting therapy. Corticosteroids may improve mood and give patients a feeling of general well-being unrelated to the effect on the disease being treated. Subjective assessments can therefore be misleading. Objective clinical parameters should be used to monitor the need for continuing or restarting therapy . proteinuria in nephritis, spirometry in asthma and creatinine kinase in myositis. Therapy should be tapered off. For example, with prednis(ol)one, the dose is reduced in steps of -5 mg every 3-7 days down to 15 mg/day. At that point, switch to alternate day therapy and reduce in mg steps over 2-3 weeks. This minimises the impact on mood and lessens the drop in general well-being.

Some parents worry that children who use inhaled corticosteroids may not grow as tall as other children. A very small difference in height and growth was found in children who were using inhaled corticosteroids compared to children not using them. 3 And one study showed a very small difference in height [about in. ( cm) ] in adults who used inhaled corticosteroids as children compared to adults who did not use inhaled corticosteroids. 4 But the use of inhaled corticosteroids has important health benefits for children who have asthma. If you are worried about the effects of asthma medicines on your child, talk with your doctor.

Nebulisers are machines that turn the liquid form of your short-acting bronchodilator medicines into a fine mist, like an aerosol. You breathe this in with a face mask or a mouthpiece. Nebulisers are no more effective than normal inhalers. However, they are extremely useful in people who are very tired (fatigued) with their breathing, or in people who are very breathless. Nebulisers are used mainly in hospital for severe attacks of asthma when large doses of inhaled medicines are needed. They are used less commonly than in the past, as modern spacer devices are usually just as good as nebulisers for giving large doses of inhaled medicines. You do not need any co-ordination to use a nebuliser - you just breathe in and out, and you will breathe in the medicine.
